At HealthLinc, we’re committed to helping everyone access affordable care, no matter their insurance status or income level.
One way we do this is through our sliding fee scale, which allows us to provide care at a discounted rate based on a patient’s household size and income.
If you’re interested in using our sliding fee scale, your first step is scheduling an appointment with one of our intake coordinators. We will help you understand your options, introduce you to HealthLinc’s services and guide you through becoming a patient.
Understanding the Sliding Fee Scale
Eligibility is based on how many people live in your household, your household’s total annual income and federal guidelines. Your intake coordinator will explain how the sliding fee scale works and answer any questions you may have.
To determine if you qualify, your intake coordinator will need to review and verify a few documents. To help this appointment go as smoothly as possible, each adult member of your household should bring the following documents with you:
- Photo ID, like a driver’s license, state ID, school ID or passport.
- Proof of income, like pay stubs from the past 30 days (if employed), last year’s federal tax return (1040) or a waiver of filing (form 4508-T). If you are employed but do not have proof, please bring a signed letter from your employer on the employer’s letterhead, including name and phone number, with the amount you are paid.
- Proof of other income, like unemployment, Social Security, pension/401(k), worker’s compensation or disability.
If you are not sure if a document will work, please reach out before your appointment.
If you qualify for the sliding fee scale, your discount will be active for one full year. Near the end of that year, you can schedule another intake visit to renew your eligibility.
Even with insurance, you can still qualify for the sliding fee scale.
Help Getting Health Insurance
If you do not have insurance, HealthLinc can help.
During your appointment, your intake coordinator can help you explore available insurance options and assist you in applying for Medicaid and marketplace plans. If you qualify for Medicare, your intake coordinator can help you schedule an appointment with one of our community health navigators who can assist you with the Medicare process.
If you do have insurance coverage, that information will be added to your patient account for your future appointments at HealthLinc.
