By following a Patient-Centered Medical Home (PCMH) model, HealthLinc can provide care that is coordinated, comprehensive and accessible in order to achieve improved health outcomes for you and your family.

Following a PCMH model allows us to put you – the patient – first in everything we do. Our goal is to make it easy to get the care you need and help you make decisions about the best healthcare options for you.

Your Care Team

Patients are assigned a medical team that provides care in a coordinated fashion. This team includes a primary care provider (family medicine physician, internist, pediatrician or family nurse practitioner), medical assistant, care team nurse, behavioral health consultant, clinical pharmacist, community health worker and care coordinator.

Having a designated care team allows you and your family to build relationships with your providers and feel more confident and empowered when asking questions and making decisions about your care.

Your care team can:

  • Provide care for all your physical and mental health needs, including prevention, wellness and acute and chronic care.
  • Assist you in navigating your health information and answer questions along the way.
  • Deliver evidence-based care that follows guidelines presented by authoritative and respected medical resources, including but not limited to: the American Academy of Family Physicians, the American Academy of Pediatrics, the American College of Obstetricians and Gynecologists, the Center for Disease Control through the National Institutes of Health, the American College of Physicians and the United States Preventive Services Task Force.
  • Offer transition care and support from hospitals and special facilities.
  • Connect you with specialty service providers.
  • Ensure after-hours care through our 24/7 phone line.
  • Offer telehealth appointments to new and established patients by phone or video call.
    • Telehealth appointments are encouraged for patients 65 and older, those with chronic conditions that weaken immunity and those who have mild infections or illnesses that may be easily spread to others.
  • Provide translation services, including interpreters and a translation phone line, so you receive care in your preferred language.
  • Ensure respect, fair treatment and equal access to care, regardless of your ability to pay.
