HealthLinc, a Federally Qualified Health Center (FQHC) with corporate headquarters in Valparaiso, IN, will assume operation of Northwest Medical Group – Pediatric Care, effective December 1, 2024.
Northwest Medical Group – Pediatric Care has two locations. On December 1, the pediatric office in La Porte, located at 1509 State St., will become HealthLinc La Porte Pediatrics and is adjacent to the existing HealthLinc La Porte clinic. Its close proximity provides easy access to expanded services, such as dental and behavioral health care, community health workers, clinical pharmacists and health insurance assistance. The pediatric office in Michigan City will relocate to HealthLinc Michigan City at 200 Alfred St., Michigan City.

Northwest Medical Group patients will continue seeing the high-quality providers and staff who have been caring for their children at Northwest Medical Group – Pediatric Care. Established pediatric patients at HealthLinc La Porte can remain at the current clinic or transfer to the new location.

As part of HealthLinc, their patients will have ready access to community health workers, behavioral health clinicians, and dentists,” said Simon Ratliff, Chief Executive Officer for Northwest Health – La Porte. “We will continue to work closely with Dr. Moustafa, Dr. Salazar and their nurse practitioners as they transition to the HealthLinc umbrella of services.”
HealthLinc has successfully integrated other clinics in the past, providing a smooth transition for staff and patients.
