Dr. Kozak provides comprehensive care to the entire family, from child to adult. He has extensive experience working with chronic disease management, such as hypertension and diabetes, and takes a special interest in the treatment of addiction, HIV and hepatitis C. Dr. Kozak is board certified in family medicine and addiction medicine.
