Dr. Michael Kozak

Dr. Kozak is a family doctor who provides comprehensive care to the entire family, from child to adult. He has extensive experience working with chronic disease management, such as hypertension and diabetes, and takes a special interest in the treatment of addiction, HIV and hepatitis C. Dr. Kozak is board certified in family medicine and addiction medicine.
Dr. Kozak received his bachelor’s degree from the University of Notre Dame, his Master of Public Administration from Indiana University and his medical degree from the University of Alabama. He completed his medical residency at Memorial Family Medicine.
