What to bring to your appointment
To help us better serve you, please bring the following documents to your appointment:
- Identification and personal documents
- Birth certificate or passport for all members of your household
- Photo IDs
- Social Security number or naturalization number for all household members
- Insurance information (if applicable)
- Medicaid, Medicare or commercial insurance card
- If you have received a denial letter from Medicaid, please bring this with you.
- Residency and household information
- Proof of current residency (e.g. utility bill, bank statement, phone bill)
- The number of members in your household
- Income verification
- Proof of income for the entire household (e.g. recent paystubs)
