Heart City Health is now part of HealthLinc

On October 1, 2026, Heart City Health became part of HealthLinc, another Federally Qualified Health Center that has been serving northern Indiana for 30 years.

You will see a new name, logo and website, but the quality of your care will stay the same.

What this change means for you:

Your clinic is now part of HealthLinc.
You will now see “HealthLinc” on your bill.
Your clinic’s name and branding will change in the coming months.
You will have more services available to you, including vision, podiatry and expanded pharmacy options.
You can continue to get care at your local clinic.
HealthLinc accepts Medicaid, Medicare, Healthy Indiana Plan (HIP) and commercial insurance. We also welcome patients without insurance.
We offer a sliding fee scale to help lower your healthcare costs.
Your healthcare team is still here to support and care for you.

Your New Patient Portal

During your first visit, you will need to sign up for HealthLinc’s patient portal. This portal lets you ask your provider questions, get important health information, get a copy of your medical record, request an appointment, pay your bill with ease and more.

To register for HealthLinc’s patient portal:

Ask a member of your care team to enroll in the patient portal.
You will receive an email with a sign-up link.
Click the link in the email to complete your registration. Please register within 24 hours of getting the email, or you will need a new link.
