Sliding Fee Scale
To help make care affordable to all, we offer a sliding fee scale to patients that qualify. This discount program is based on household size and income and can be used even if you have insurance.
Financial Assistance Options for Families & Individuals
To take advantage of our sliding fee scale, we will need some information from you at your intake appointment. If you don’t bring all the information, we will need to reschedule your intake appointment. If you have questions, call us and ask to speak with the intake coordinator you are scheduled with.
To apply for the sliding fee scale, please schedule an intake appointment and bring the following documents:
- Photo identification (driver’s license, state ID, school ID) if you are not already a patient
- Household size
- Proof of current income
- Proof of other sources of income: unemployment, Social Security, pension/401(k(/annuities, workers’ compensation, disability, self-employment profit or loss, etc.
If you (or someone in your household) works but does not have proof, please discuss this with our intake specialist during your appointment.
