Junk Mail – Medicare Assistance Event

Do you get Medicare information through the mail that is hard to understand? HealthLinc can help!

Starting October 18 until October 22, you can bring your mail to HealthLinc Valparaiso and meet with a SHIP counselor and pharmacist for help picking the plan that is right for you! Walk-ins are welcome, but we recommend making an appointment by calling 1-888-580-1060 Ext. 2461.

Please bring the following items with you on the day of your appointment:

• Your Medicare card
• Your current prescription medication in the bottle with labels OR a complete list of your medication names and dosages
• Your Medicare prescription drug plan current card (If you are already enrolled)
