In recognition of HealthLinc’s services in Valparaiso, the National Committee for Quality Assurance (NCQA) has endorsed HealthLinc’s Valparaiso Clinic as a Patient-Centered Medical Home (PCMH) provider. “The NCQA Patient-Centered Medical Home is a model of 21st century primary care that combines access, teamwork, and technology to deliver quality care and improve health,” said NCQA President Margaret E. O’Kane. “NCQA’s PCMH Recognition shows that HealthLinc’s Valparaiso clinic has the tools, systems, and resources to provide their patients with the right care at the right time.”
HealthLinc CEO Beth Wrobel believes that this distinction exemplifies the vision of HealthLinc to be known for its compassion and quality of care. HealthLinc services are truly patient-centered through a team of care providers in each clinic. With physicians, nurses, behavioral health consultants, dentists, and other medical-related staff, HealthLinc is able to care for the whole patient and family. HealthLinc’s continuity of care ensures that patients can receive access and advice during and after hours, open scheduling, expanded hours, and new options for communication between patients, their personal provider, and practice staff through the patient portal.
To our knowledge at the time of the award, HealthLinc:
- is the 1st primary care office in Northwest IN to be recognized by NCQA
- is the 3rd Community Health Center in the state to be recognized as a PCMH
- employed 20% of the providers recognized by NCQA as PCMH providers in the state
In addition, to the above accomplishments, HealthLinc continues to be 1 of 500 Community Health Centers participating in the Medicare APCP Demonstration Project, encouraging and monitoring the effectiveness of medical homes in our communities.
