Katherine A. Lisoni, M.D.
Dr. Katherine A. Lisoni is a practicing Family Physician and Mishawaka’s Site Medical Director. She earned her Master’s degree in Public Health from Northwestern University, followed by her Medical Doctorate from its Medical School. Her residency was completed at St. Joseph’s Hospital in South Bend, followed by a one-year Fellowship in International Health specializing in global health focused on obstetrical care, emergency dental procedures, and minor surgical procedures. Fluent also in Spanish, Dr. Lisoni is board certified by the American Board of Family Medicine and has privileges at St. Joseph Regional Medical Center.
Kathryn Plummer, RN, CNS, NP-C
Joining HealthLinc in 2011, Kathryn Plummer fulfills the role as a Family Nurse Practitioner. She received her Bachelor’s degree in Nursing at Indiana University South Bend. After earning her Master’s degree in Nursing with a Gerontologial Clinical Nurse Specialist major, she later returned for a post-master’s Certification for a Family Nurse Practitioner. Kathryn is board certified by The American Academy of Nurse Practitioners, and her previous background included 5 years of experience in a large hospital, serving multiple floors and units.
Daniel J. Triezenberg, M.D.
Joining HealthLinc in 2012 as a Physician, Dr. Daniel Triezenberg has almost 40 years of experience in family medicine. His Medical Degree was earned at the University of Michigan Medical School and his residency completed at the Medical University of South Carolina. He currently has privileges at St. Joseph Regional Medical Center in South Bend, where he also serves as the Associate Director of the Family Medicine Residency Program. Dr. Triezenberg is board certified by the American Board of Family Medicine, the American Academy of Family Practice, and the National Board of Medical Examiners. In addition to his many professional memberships, his experience also includes teaching and publishing research articles in his field.
Judith Weaver-Yoder, RN, CNM
Joining HealthLinc’s clinical staff as a Certified Nurse Midwife in 2012, Judith Weaver-Yoder has 30 years of experience in maternity and women’s health and almost 20 years in midwifery. She holds degrees in Nursing and Social Work from Goshen College and a Master of Science in Nursing from Case Western Reserve University. Her certification in Nurse-Midwifery is from Frontier Nursing School of Midwifery & Family Nursing, and she is board certified by the American College of Nurse Midwives. In addition to teaching classes in women’s health, she is also certified as a Professional Legal Nurse Consultant (PLNC).
Rachel E. Whitcomb, RN, CNM
Rachel Whitcomb has been a Certified Nurse Midwife with HealthLinc since 2013. She received her B.S. degree in Nursing from St. Mary’s College at Notre Dame and her Master’s in Nurse Midwifery from the University of Illinois at Chicago. Rachel is board certified by the American Midwifery Certification Board and is a member of The American College of Nurse Midwives. In addition to 12 years of midwifery experience, her background also includes teaching in the Nursing Department at St. Mary’s and serving as a Lieutenant in the Navy Nurse Corps.
Milcah Gichuru, RN, NP-BC
Certified by the American Nurses Credentialing Center, Milcah Gichuru joined HealthLinc in 2012 as a Family Nurse Practitioner. Originally from Kenya, her Bachelor’s degree in Nursing was completed at Indiana University, South Bend, and her Master’s degree in Community Health Nursing was earned from the University of Michigan, Ann Arbor. Milcah’s professional experience also includes six years as an RN and Nurse Technician at St. Joseph Regional Medical Center and 26 years as a Medical Laboratory Technologist.
Thomas Keller, LCSW
Thomas Keller serves as a Behavioral Health Consultant as part of HealthLinc’s integrated health program. Also on staff at Oaklawn as a licensed clinical social worker, Thomas received his B.A. degree in Criminal Justice and Psychology from Indiana University and his Master’s in Social Work from East Carolina University. He has more than 20 years of experience in his field working with addictions, abused and neglected children, and emotional and behavior disorders.
