Milcah Gichuru, FNP
Milcah Gichuru, FNP, is a family nurse practitioner at HealthLinc’s Mishawaka Clinic. With over 600 hours of clinical rotation in the areas of family health, adults and geriatrics, pediatrics, and women’s health, Milcah provides comprehensive care for a wide variety of patients with diverse needs. Milcah’s Family Nurse Practitioner Master’s degree was completed at the University of Michigan, Ann Arbor, with undergraduate studies at Indiana University. In addition to practicing at HealthLinc, Milcah is an RN on staff at St. Joseph Regional Medical Center in Mishawaka.

Site Medical Director
Dr. Katherine A. Lisoni, MD, is a practicing Family Physician and the Site Medical Director at HealthLinc in Mishawaka. She completed her Medical Doctorate from Northwestern University Medical School and graduated from St. Joseph Family Medicine Residency program in South Bend. She also completed a Fellowship in International Health through her St. Joseph Family Medicine Residency, specializing in global health with a focus in obstetrical care, emergency dental procedures, and minor surgical procedures.
Kathleen Long, NP
Debra Neubauer, NP
With over 30 years in the nursing field, Debra Neubauer provides experience and compassion to all of her patients. Most recently, she worked at Elkhart General Hospital as a Medical-Surgical Nurse and at University of Texas Southwestern Medical Center. She has an extensive nursing background at all levels from bedside care to upper management and is a valuable asset to the HealthLinc team!
Michelle Streb, NP
Michelle Streb attended Indiana University and Goshen University for her Bachelor of Science in Nursing and Masters in Nursing, respectively. She holds an array of certifications and distinctions. With nearly 30 years of nursing experience, Michelle has joined HealthLinc’s team as a Nurse Practitioner.
Dr. Daniel J. Triezenberg, MD
Joining HealthLinc in 2012 as a Physician, Dr. Daniel Triezenberg, MD, has almost 40 years of experience in family medicine. His Medical Degree was earned at the University of Michigan Medical School and his residency completed at the Medical University of South Carolina. He currently has privileges at St. Joseph Regional Medical Center in South Bend, where he also served as the Associate Director of the Family Medicine Residency Program. Dr. Triezenberg is board certified by the American Board of Family Medicine, the American Academy of Family Practice, and the National Board of Medical Examiners. In addition to his many professional memberships, his experience also includes teaching and publishing research articles in his field.
Judith Weaver-Yoder, RN, CNM
Judith Weaver-Yoder serves as a Certified Nurse Midwife at HealthLinc’s Mishawaka location. A member of the American College of Nurse Midwives for two decades, she has been a practitioner and manager of Midwifery of Michiana since 1995. She holds degrees in Nursing and Social Work from Goshen College in Goshen and a Master of Science in Nursing from Case Western Reserve University. Her certification in Nurse-Midwifery is from Frontier Nursing School of Midwifery & Family Nursing.
Rachel E. Whitcomb, RN, CNM
Rachel Whitcomb, CNP, NPW, MS, RN, is a certified midwife at the Mishawaka clinic. Between her full scope midwifery, nursing and teaching as an assistant professor at Notre Dame, Rachel is full of wisdom and provides well-rounded care for her patients. Her education includes the University of Illinois at Chicago and St. Mary’s College at Notre Dame.
Sandy Imanse, LCSW
Sandy Imanse has over 25 years of social work experience. She has a very diverse background within her field, with positions ranging from mental health therapy to a public school guidance counselor. Sandy earned her Bachelor of Social Work from Western Michigan University and her Master of Social Work from Michigan State University. She comes to HealthLinc excited to make a difference to our patients in Mishawaka.
Thomas Keller, LCSW
Thomas Keller serves as a Behavioral Health Consultant as part of HealthLinc’s integrated health program. Also on staff at Oaklawn as a licensed clinical social worker, Thomas received his B.A. degree in Criminal Justice and Psychology from Indiana University and his Master’s in Social Work from East Carolina University. He has more than 20 years of experience in his field working with addictions, abused and neglected children, and emotional and behavior disorders.
