Valparaiso, IN – HealthLinc is proud to announce that all of its clinics have received national distinction by the National Committee for Quality Assurance (NCQA), a nonprofit dedicated to improving health care, for providing patients high quality, coordinated health care and access.
The HealthLinc Mishawaka, Knox, Michigan City, East Chicago and Porter Starke clinics join the Valparaiso clinic in receiving this honor.
Through its Patient-Centered Medical Home (PCMH) Recognition Program, NCQA distinguishes health care organizations and providers that deliver high-quality care by increasing patient access, utilizing the latest in health information technology, and promoting partnerships between clinic staff and patients.
A patient-centered medical home is a model of care that strengthens the physician-patient relationship by replacing episodic care with coordinated care and a long-term, physician-patient relationship.
